On January 1, 2011 Obstetrics, Gynecology and Infertility has merged with three OB/GYN clinics.  Our clinic name, physicians, nurse practitioners and clinic locations will not change.  The physicians and nurse practitioners will continue to be participating providers of all the health plans listed in the insurance section of our website.  The only difference is that our clinic will be a division of Obstetrics and Gynecology Associates, PA.  If your health plan lists physicians/NPs by clinic name, then you may find their name under Obstetrics and Gynecology Associates and/or Obstetrics, Gynecology and Infertility. If you have any question on insurance participation, please call our business office at 952-567-7050. 

Due to the complexities and regulations in the insurance market, we depend upon the patient to determine whether your insurance company does or does not cover a specific service or procedure. Please understand that we cannot accept responsibility for payment or nonpayment of your insurance claims, nor can we negotiate a settlement on a disputed claim. Questions about coverage and benefits are between you and your insurer.

Plans that require a referral for you to be seen by one of our physicians or nurse practitioners are noted as such below. If your insurance company requires a referral, please call your primary care physician to obtain it before you schedule an appointment with our practice.
