Please call your insurance company to determine whether your insurance company or care system/network is contracted with these hospitals or the surgery center prior to your treatment.

For Example: Fairview Physician's Associates (FPA) does not contract with North Memorial Medical Center or West Health Outpatient Surgery Center. They are contracted with the Fairview System Hospital.

First Health insurance has contracted with Fairview Southdale Hospital. It does not have a contract with North Memorial Medical Center or West Health Outpatient Surgery Center.
